Job Description

The Rapid Rehousing Specialist is part of our Home Soon/Rapid Re-housing Initiative and is responsible for assisting clients in finding appropriate housing placements, providing educational support designed to increase housing retention, providing housing advocacy as needed and providing aftercare services that help to support retention in permanent housing. 

 

Brief description of the essential functions:

  • Provide supportive and empathic individual housing search sessions with clients
  • Develop, implement and facilitate housing groups
  • Develop and coordinate a system for follow-up/outcome measurement for up to 12 months post discharge.
  • Formulate and document individualized housing plans for clients who are receiving intensive housing search coordination. 
  • Obtain signed consent to support and empower clients with the coordination of referrals and housing search
  • Conduct outreach to potential landlords.
  • Maintain an up to date housing resource database.
  • Maintain effective case management documentation, record keeping systems and electronic health records.
  • Serve as a liaison between external / internal providers and our residents

 

Qualifications:

  • Bachelor’s degree (preferred) with significant administrative experience including networking.
  • 2 years of case management experience.  
  • Access to reliable transportation in order to provide in home visit to stabilization clients.
  • Bilingual (English/Spanish) preferred.
  • Proficient computer skills, including the use of Microsoft Word and Excel.
  • Sensitivity to addiction and recovery, cultural heritage, gender, sexual orientation, race, religion, medical and psychiatric conditions.
  • Strong organizational, writing and communication skills.

 

Days/Schedule: 

  • Full-time position: 40 hours/week (Monday - Friday, 9am-5pm or 10am-6pm) with flexibility for doing some off hours work to look at units and meet landlords

 

Rate: $21.30/hr
