Nurse - Health Care Coordinator

• The Behavioral Health Home managed by Bay Cove Human Services is a program that provides comprehensive care coordination to individuals with chronic conditions, including a behavioral health concerns.
• The model operated at Bay Cove is the Coordinated Care Model, where staff ensures members are receiving the medical and behavioral health services they need.
• Bay Cove Health Home staff break down their work into four broad categories: assessment, care planning, authorization management, and care coordination.
• The RN Care Partner will act as a liaison between the health insurer, Commonwealth Care Alliance, members of the clients care team, and Bay Cove providers.
• They will provide community based nursing for an identified caseload of members living in the community with developmental and psychiatric disabilities and complex medical needs.
• Care Manager is responsible for assessing the needs of members, making referrals for services and authorizing services.
• The RN Care Manager also acts as a medical resource for the Bay Cove Human Services Behavioral Health Home interdisciplinary team.
• No holidays or on call responsibilities.

Qualifications:

• Registered Nurse licensed in MA.
• 3-5 yrs experience.
• Experience working with people diagnosed with chronic mental illness and complex medical conditions.
• 1-2 yrs experience with acute long term care.
• Home care experience preferred.
• Driving License Required
