Job Description

We are looking for people to partner with us to achieve our goal of disrupting the cycle of homelessness for greater Boston families and are seeking a Case Manager  to join our team. 

 

What You Will Do:

  • Conduct psychosocial needs assessments of the head of household and formulate action plans as needed.
  • Help families navigate barriers to tenancy and build a support system by connecting them with people and programs in the community. 
  • Provide support to a family in a moment of crisis and report significant concerns to program management in a timely manner.
  • Utilize clinical consultation support, as needed, in order to access the individual’s or family’s level of need.
  • Assist in the development and facilitation of groups, classes, and skills training to help clients achieve goals and specifics within the action plan set.
  • Measure and review case progress and facilitate case consultation meetings.
  • Work as part of a team and coordinate with Housing Specialists, System Navigators, Residential Assistants, Supervisors and other program staff.
  • Work onsite five days a week at multiple locations.
  • Participate in on-call duties. 
  • Participate in the on-call rotation.

What You Will Need to Bring:

  • Education – Associate Degree, Bachelors preferred. A combination of education and experience will be considered.
  • Experience – 3 to 4 years of experience in case management, housing search or related work with homeless or low-income families. Detailed understanding of Massachusetts benefit programs, state and federally funded housing programs, private housing developments and the social service network. Residential program experience is a plus.
  • Skills – Ability to work effectively with individuals from a wide range of ethnic and social backgrounds is critical. Must be computer literate, familiar with MS Word and competent to utilize a variety of software programs necessary for reporting requirements. Must be able to communicate effectively verbally and in writing. Ability to ambulate stairs unassisted to conduct home visits.
  • License/Certification – Must be CPR/SFA Certified within 30 days of hire
  • Language – Bilingual in Spanish/Haitian Creole preferred
  • Additional Information- Use of a personal vehicle or ability to efficiently utilize public transportation for home visiting is required.
